In the meantime I found the problem which is in fact a glitch in Word 365.
When using a simple docx with some pictures with alt-text and saving as PDF there is a problem.
I use Word 365 and use "Save as ..." and select in the drop-down "PDF".
When checking this PDF in Acrobat Pro 2017 the reflow is not working due to an error in the <Figure>Tag (Tags pane).
It is missing the objectcodes. After re-coding in Acrobat pro you can solve the problem in the PDF.
Making the PDF via the plugin "PDFmaker" works fine in this respect.
