Got a similar situation got a employee who is unable to use the request for signature on her desktop app. Have done the following
- checked for Adobe updates.
- repaired adobe already
- uninstalled and reinstalled it.
- still no avail. Screen is just white after hitting request for signatures.

only way could get it working by signing into adobe on the web browser. This is just consider a workaround not a proper fix.
