   For PDFs having active form fields, only the sign option is displayed. This is AsDesigned behavior.

   The author probably didn't set it up right. If you have the same issue as the guy with the W-9, I would fill the parts you can, save as a jpeg, open in word or something similar, add a text box over the image, save as pdf. It's round about, but at least you don't have to print and sign.
