Hello,
Thanks for reporting this issue.
But we are not able to reproduce this issue at our end.
Thus please let us know your Acrobat version and OS details. Moreover, are you facing this issue for every file? If not, share the sample file.
Thanks,

Here are links to two test files that you can use to duplicate this error:
https://www.jt-architecture.com/bidding/photos/blank.pdf
https://www.jt-architecture.com/bidding/photos/123.pdfblank.pdf -- this one is blank but it has a button field in it
123.pdf -- a three-page file with 1, 2, and 3 on the pages so you can see which is which. If you open up blank.pdf, select the button using the button edit tool, and select properties you can then select an icon for the button. If you choose 123.pdf as the file for the icon image, you will be able to select page 1 but it will display in the button as page 2. You can also select page 2 and 3 correctly, but never get page 1 to display in the icon as it should.

I posted the original report of this bug -- it still occurs even with the latest update of Acrobat 2017 (17.011.30148)
Attached are two files -- one called "button page" that has a button on it (with icon) and a second PDF called "icons - 3 pages" that has three different pages in it. I can get the second or third page of this "icons" file to show up as the button icon, but I cannot select the first page as the icon (you can select it in the dialog box, but it shows up as the second page).
I am using Windows 10, latest version all updated.
