   I am on a Retina 5K iMac (27-inch, late 2015), 4 GHz Intel Core i7, 64 GB memory,
   macOS Sierra, 10.12.6.

   Yes. This problem is still happening. At least it is still happening with the Pan & Zoom panel.
   It is not happening with other panels, such as the Loupe tool or the Reading Order panel.

   That issue was so long ago and I know we figured some explanation at the time. However, we have never (luckily) experienced this problem further. And I cannot remember what we did to solve this.
   Anyway, thanks for your response!
