"Add note to text" was there in the Quick Action Toolbar from the first time. Unfortunately, Adobe removed it in subsequent updates.
I really can't understand why this useful feature was removed!

Please, allow me to share my custom toolbar with you, in case U find it useful.
You can use it as a template that U can edit as U wish.
Furthermore, I heard a lot of customers complaining about the right hand tool pane that takes up so much screen space. I have taken it out from my customized tool bar. I hope U enjoy it.

Here are the steps U follow to import it & edit it if U like:
    1. Download the file
    2. Double click it to import to Acrobat DC. But if U have Acrobat XI installed on the same machine this may not work. So, skip to step 3
    3. Open Acrobat DC and click on the wrench icon on the right or click the tools tab at the top left corner.
    4. Scroll to the bottom of the tools & click on the drop down arrow of the "Create custom tool" button
    5. Click "Import" & navigate to the location U downloaded the customized tool bar to and import it
    6. Close Acrobat & open it again
    7. Go to the tools menu by clicking the "Tools" button at the top left corner
    8. Scroll to the bottom and U will find the Customized tool I created. It is named "DCSet"
    9. Go to the "DCSet" and click "Add". Voila! U now have it added to the tool pane on the right. But thankfully even when U click on it to add the tools set to the tool bar, the tool pane remains collapsed.
    10. If U want to edit the DCSet, go to it and click the drop down arrow and select "Edit"
    11. Add and remove whatever icon U with. But be careful not to add anything to the tool pane on the right, only add icons to the tool bar at the top.

@Kelly Vaughn. I think U missed his quest. Ur video teaches how to customize the toolbar & tool pane, but I think U missed the real point. The question is how to keep the customized toolbar/tool pane always on, pinned to the toolbar.

@Ayush Jain. Thanks. I followed your 7 steps to creating a customized toolbar. It was successful! However, I cannot find the "Pin functionality" that you mentioned. Because of that each time I have to click on the shortcut to the customized toolbar in order to open it.
Please, how do I go about pinning my customized toolbar as my default toolbar, so that each time I open a pdf file it will be there?
    Thanks

Yes, kindly move the "Home", "Tools" and "Documents" buttons to the menubar, where there is ample space doing nothing (please see the attached image to see what I am suggesting)!
This will create more space for the workspace.

I fully agree with the suggestion. But wait, what does it take for Adobe to bring back the cute "Add note to text" button? It is so useful.
I learned that Adobe removed it because they thought it is redundant, and that the highlight tool is enough. But apparently the tool is so useful to many.
I hope Adobe brings it back, so that those that find it useful can still enjoy it. Those that find it redundant can simply ignore it!

    @Ayush Jain.
Thanks for your suggested 3 steps to setting up "customized quick tools list".
I followed your instructions. I was able to customize the toolbar. But, as soon as I click on an icon I dragged from the edit menu, my customized toolbar is replaced by Edit toobar. Similarly if I select an icon I dragged from the "Organize Pages" toolbar, e.g. extract button, my customized toolbar gets replaced by the "Organized Pages" toolbar with all the other buttons e.g. insert, extract, delete page etc. I am only interested in just one button out of the whole barrage!
Now another frustration is after I am done with the feature I need, I will have to click on my added customized toolbar icon before I can get it back. So, if I use tools from different toolsets 1000/day I will have to click on my customized toolbar button 1000 times!
Please, can Adobe make my Cutomized toolbar as the default that remains fixed in place, unless I want to change it. Otherwise, there is little use creating a customized toolbar that keeps on disappearing.
    Thanks for your time
