I am Unable to INSERT an image on your COMBINE, EDIT, FORM. THE INSERT ICON is GREYED OUT

We need more information on steps to be able to reproduce this issue.
Please share the version of acrobat you are using. You can do so using the link -helpx.adobe.com/acrobat/kb/identify-product..
And also share the test PDF file using forums.adobe.com/docs/DOC-7161 for us to investigate further.
