Combining similar pdf forms

I have a fillable pdf form which is used for different activities, so the form remains the same but the information is different and several forms need to be combined into the same pdf document.

    For me, i have a transmittal sheet in a form (1) that i write list of material to be transferred. Once i run out of room, i want to attach another page onto this form (1). So i want Form (1) followed by Form (1) again in one pdf. Issue is that since they share the same data for forms, when i fill out the first page, the second page copies the same information. Instead, if and when adding the same type of document to the first one, if it can make the information for the form fields unique, then it would work more smoothly rather than having 2 separate pdfs.
