form template
I need to send in CMS-1500 forms on a regular basis. I would like a template to be able to do this. The only thing that has to change on the form are some of the dates and payment amounts. Can I use a template, fill in some of the fields, send to sign and then recover the template for the next use.

that is totally possible yes. I do things like that all the time.
create PDF, then "Prepare form" - Full Acrobat of course.
