Acrobat/Reader should support dynamic DPI scaling.
We work on multiple monitor in our office. Our monitors have different scaling factor settings. When we drag Acrobat window from one display device to another it doesn't scale as per the new scaling factor.
Fix for scaling issue is currently available for following tracks: Acrobat DC Continuous, Acrobat 2017, Acrobat Reader DC Continuous, and Acrobat Reader 2017

Thank you Louis for posting your fix on 21 Feb 2019. Works perfectly for me (Acrobat Pro 11 on Windows 10 on a Surface Pro linked to a Dell Ultrasharp monitor 2560 x 1440 pixels).
Fix instructions repeated here for convenience of others:
Open the Adobe program folder (mine is Windows C: > Program Files (x86) > Adobe > Acrobat 11.0 > Acrobat)
Scroll down to Acrobat.exe. Right click.
Select Properties and then the Compatibility tab.
Select 'Change high DPI settings' button.
Check the box next to: Override high DPI scaling behavior.
From the dropdown below select 'System (Enhanced)'. (or try 'System' only if that doesn't work).
Select OK.
Select Apply. Select OK. -

Found a fix that worked for me:

Hi, I had the same problem a few months ago on a Surface Book but one of the updates fixed it. Noy sure if MS or Adobe. I am now on Acrobat DC (18.009.20050)

I have Adobe Acrobat Pro XI v 11.0.23, using Windows 10 Pro on a Dell XPS 13 hi-res laptop. When I open Adobe the icons and the menu are so tiny they are unreadable, as are any comments. Why is Dynamic scaling not also implemented for Acrobat Pro XI?? The program is not usable for me, and Adobe Support is no help. What is everyone else doing to make this functional at all for them??

The only version of Acrobat Pro available to me on a federal government computer is XI... Will this fix also be implemented for Acrobat XI?

It says no updates available. So I am stuck, right?

We released an update that fixes scaling issues. This update will be automatically applied to all Acrobat and Reader users in the coming days. You may also update to the latest release by opening the application and going to Help > Check for Updates.

Yes me too - very painful when I open a PDF .... as the Display Tool and Menu bars are massive - there must be a fix - can anyone help?

Same problem. My laptop screen is 4k (3840 x 2160) and my attached monitor resolution is 1920 x 1080). The program looks normal on my laptop screen but doesn't adjust when I drag it to my other monitor. Other programs by most other vendors will detect the screen resolution and adjust. Adobe needs to do this as well.

When using an external display Acrobat DC does not scale properly when dragging windows between displays. It is not a screen resolution issue as I do not have this problem with any other applications and Adobe tech support just spent an hour troubleshooting this to no avail. I am running Windows 10 on a 3200 x 1800 laptop with a 1920 x 1080 external display. Please help.
