Forms being returned to me blank

Created my form using Adobe DC/Pro. I send to my clients to complete, they complete it, but when they return it to me, I get it back blank

To investigate further, please help us with following

1. How are you sending these forms for filling - via email or via servers.?
2. Are the client using browser or Apple Preview to fill these forms?
3. Are you receiving these filled forms as attachment?
4. Do you observe this issue on Win and MAC both or only on one of these OS?

I get this problem too about 25% of the time.

I got that too. I can't even preview the document after I have put the pdf into a folder. Just says "please wait....." What can I do to view this?
