This report references InDesign Bug ID – ID-4204915 Bullets are voiced twice – posted by Bevi Chagnon on September 22, 2017, 6:53 AM.
Problem Description:
When a different bullet is used in In Design other than the 3 default bullets (round, asterisk, and lozenge), the tag tree in the exported accessible PDF has two span tags, one empty and the other with the bullet character.
The double span tags cause screen readers to voice this as "bullet bullet" (or whatever glyph is used) when it should be just "bullet." It produces a double voicing
Steps to Reproduce:
1. Make a bullet style in InDesign.
2. Set it to use any Unicode character, other than the standard round bullet, asterisk, or lozenge.
3. Export to PDF.
4. Test with a screen reader (JAWS, NVDA, etc.) and the bullet will be voiced twice.
The extra span in the <Lbl> tag.

Expected Result::
Bullet blah blah
Bullet blah blah
Any Workarounds:
1. In Acrobat Control panel, select and artifact offending “empty” span containers.
2. In Tags panel, select top Tags and right-click to Delete Empty Tags.

Hi Rachit,
I believe that's an incorrect assumption.
This is a problem with how InDesign's bullet/number style is exported with Adobe's Export To PDF utility. It is not correctly interpreting the bullet character.
This is NOT a problem with what the user is doing in InDesign; after all, there's not much they can do in InDesign.
The bug is NOT fixed.
